We’re partnering with a well-respected Not-for-Profit aged care provider that is truly committed to supporting older people to live independently and with dignity in their own homes. With a strong values-based culture and supportive leadership team, they are now looking for an experienced Care Coordinator to join their community team on a full-time basis.
This role will suit someone who understands Home Care Packages, is confident with budgets, and enjoys building relationships with older people, families and care providers.
The Opportunity Working as part of the Home & Community Care team, you will:
  • Coordinate services for older people receiving Home Care Packages, ensuring support is aligned to their assessed needs and goals
  • Set up and onboard new clients, including service arrangements and care documentation
  • Manage and monitor HCP budgets, ensuring funds are allocated appropriately and used effectively
  • Schedule services and manage referrals in consultation with Care Partners, clinicians and the wider support team
  • Maintain accurate client records, service logs and appointment information in the client management system
  • Monitor changes in client needs, escalating risks or concerns as required
  • Provide professional, empathetic communication to clients, families and providers
About You To be successful in this role, you will bring:
  • Proven experience working with Home Care Packages (HCP) in a coordination or case management role
  • Demonstrated experience in HCP budget management, tracking spending and approving services
  • Previous experience working with older people in home care, community or aged care services
  • Strong administration and documentation skills, with confidence using client management systems
  • An understanding of person-centred and goal-directed care planning
  • Excellent communication skills and the ability to build rapport with older people, families and service providers
  • Certificate III or IV in Aged Care, Individual Support or similar, or equivalent experience
  • Current Victorian Driver’s Licence
