Care Partner / Case Manager – Aged & Community Care
Full-Time | 6-Month Contract to Permanent | Gisborne VIC

We are partnering with a well-respected Not-for-Profit organisation known for delivering genuine, high-quality care to older Australians in the community. This is an excellent opportunity to join a supportive and collaborative team who are passionate about client-centred care and making a meaningful difference.
This is a full-time, 6-month contract with the view to permanent, based in Gisborne and working on-site.

About the Role
As a Care Partner / Case Manager, you will provide high-quality case management and customer service to clients and their representatives. You will complete assessments, develop care plans and budgets, and support clients to understand and manage their Home Care Package funding. You will ensure services are delivered in line with guidelines while upholding client rights and promoting independence and wellbeing.

Key Responsibilities
  • Conduct respectful client intake and assessments over the phone
  • Develop care plans, budgets and reviews tailored to individual needs
  • Provide clear information regarding funding, entitlements and service options
  • Maintain accurate case notes and communicate effectively with internal and external stakeholders
  • Promote and refer clients to appropriate services
  • Support quality improvement and best-practice care delivery

About You
  • Experience working within Home Care Packages or Aged Care is essential
  • Knowledge of Support at Home reforms
  • Understanding of CHSP, HCP and Support at Home guidelines and funding
  • Experience using the LOOKOUT system highly regarded
  • Certificate III (minimum) in Allied Health, Community Health or Nursing
  • Current First Aid Certificate and Driver’s Licence
  • Strong communication, documentation and client engagement skills
