We are partnering with a well-respected Not-for-Profit organisation that prides itself on providing genuine care and quality support the elders. This is an opportunity to join a passionate and supportive team who truly value collaboration, compassion, and client-centred care.
Working part-time, office-based , you’ll be part of a dedicated team delivering Home Care Packages and other community-based programs that make a real difference in people’s lives.

About the Role
As a Care Partner / Case Manager, you’ll provide high-quality case management and customer service to clients and their representatives. You’ll complete assessments, develop care plans and budgets, and support clients to understand and manage their package spend. You’ll also ensure services are delivered within guidelines, upholding client rights and promoting independence and wellbeing.
Key Responsibilities
  • Conduct respectful client intake and assessments over the phone
  • Develop care plans, budgets, and reviews that meet individual needs
  • Provide clear information to clients on funding and available choices
  • Maintain accurate case notes and effective communication with stakeholders
  • Promote and refer clients to a range of services
  • Support continuous quality improvement and best-practice care models

About You
  • Experience working with Home Care Packages or Aged Care is essential
  • Knowledge of Support at Home reforms
  • Understanding of CHSP, VHC, and Support at Home guidelines and funding
  • Experience using the LOOKOUT system
  • Certificate III (minimum) in Allied Health, Community Health or Nursing
  • Current First Aid Certificate and Driver’s Licence
  • Strong communication and case noting skills, with a client-first approach
