Morgan Consulting is partnering with a well-respected Not-for-Profit organisatio n known for delivering genuine, high-quality care and support to elders within the community. Working part-time (22.5 hours per week across four days) in an office-based role, you’ll be part of a dedicated team delivering Home Care Packages and other community-based programs that make a real difference in people’s lives.
About the Role:
As a Care Partner / Case Manager, you’ll provide high-quality case management and customer service to clients and their representatives. You’ll complete assessments, develop care plans and budgets, and support clients to understand and manage their package spend. You’ll also ensure services are delivered within guidelines, upholding client rights while promoting independence and wellbeing.
Key Responsibilities
- Conduct respectful client intake and assessments over the phone
- Develop care plans, budgets and reviews tailored to individual needs
- Provide clear information to clients around funding and available choices
- Maintain accurate case notes and communicate effectively with internal and external stakeholders
- Promote and refer clients to appropriate services
- Contribute to continuous quality improvement and best-practice care models
- Experience working with Home Care Packages or Aged Care (essential)
- Knowledge of Support at Home reforms
- Understanding of CHSP, VHC and Support at Home guidelines and funding
- Experience using the LOOKOUT system
- A minimum Certificate III in Allied Health, Community Health or Nursing
- Current First Aid Certificate and Driver’s Licence
- Strong communication and case-noting skills, with a genuine client-first approach
