About the Role
We are seeking a Client Advisor / Case Manager to join the team on a temporary full-time basis. Based in Gippsland, Warragul, or Traralgon, you will be responsible for managing a portfolio of clients receiving Home Care services. This role involves working closely with clients, families, and internal teams to develop tailored care plans that support independence and enhance quality of life. Flexible working arrangements may be available for the right candidate.

Key Responsibilities

  • Conduct client assessments and develop personalised, goal-focused care plans
  • Provide guidance on Home Care Package budgets and available services
  • Liaise with carers, families, and service providers to coordinate care
  • Ensure services meet Aged Care Quality and Safety Standards
  • Support and mentor in-home care staff
  • Identify opportunities for service improvements and best practices
About You
To be successful in this role, you will have:
  • Experience in case management within aged care, community services, or a related sector
  • Strong knowledge of Home Care Packages and aged care legislation
  • Excellent communication and relationship-building skills
  • A client-focused approach with the ability to problem-solve and prioritise effectively
  • Tertiary qualification in Health, Social Services, or a related field (preferred)
  • A valid Driver’s License and a current Police Check (or willingness to obtain)
