Care Partner / Care Coordinator – Community Aged Care
Not-for-Profit Aged Care Organisation | Blackburn VIC
6-Month Contract (Temp) with Strong Potential to Go Permanent

Morgan Consulting is partnering with a well-regarded Not-for-Profit organisation within the Aged Care sector, based in Blackburn. Due to growth and internal demand, they are seeking an experienced Care Partner / Care Coordinator to support their community services team.

This position will commence as a 6-month temporary contract, with strong potential to transition into a permanent role once probation has been successfully completed.

This is a busy, fast-paced role supporting clients within the community and is best suited to someone who genuinely enjoys providing high-quality service, understands aged care funding models, and is confident working with budgets and service coordination.

About the Role
Working closely with internal teams and key stakeholders, you will support service delivery for clients receiving community-based aged care services. Key responsibilities include:
  • Supporting client intake, assessments and onboarding
  • Coordinating services and maintaining client documentation and records
  • Assisting with budgets and purchase requests to support package spend
  • Acting as a key point of contact for clients and their families, responding to enquiries and providing clear updates
  • Supporting service delivery across Home Care programs while ensuring compliance and client rights are upheld
  • Identifying service gaps or risks and escalating as required
  • Working collaboratively with internal teams to ensure best practice and quality outcomes
Heavy Must-Haves (Non-Negotiable)
To be considered for this role, you must have:
  • Proven experience in Community Aged Care (essential)
  • Strong understanding of CHSP funding and Support at Home (SAH) funding models
  • Demonstrated experience working with budgets and package spending or knowledge and understanding.
  • Strong customer service and communication skills (this role is highly client-facing)
